Job description
St. Croix Hospice is seeking a Registered Nurse Case Manager to deliver holistic, in-home hospice care in Mason City, IA. You will collaborate with a care team to develop and update care plans, conduct comprehensive assessments, and manage symptoms with empathy and clinical excellence.
You will educate patients and families, participate in meaningful end-of-life conversations, and rotate on-call duties while ensuring smooth transitions and quality of life for patients and their loved ones.
