Job description
Gentiva Hospice in Mississippi is seeking an experienced Hospice RN/Case Manager to lead patient and family care at home or in alternative settings. You will perform assessments, develop care plans, and coordinate with physicians, LPNs, CNAs, social workers, and chaplains to ensure compassionate, evidence-based end-of-life care.
You will educate caregivers, monitor symptoms, and document care while maintaining regulatory compliance and Infection control standards.
