Job description
Legacy Hospice is seeking a compassionate Registered Nurse Case Manager (RNCM) to deliver high-quality end-of-life care to patients in their homes in Poplar Bluff, MO. You will coordinate care with an interdisciplinary team, administer medications, and educate families on symptom management and care planning.
The role requires an active RN license in the state, 1 year of hospice/home health experience, CPR/BLS, and a valid driver’s license. A BSN is preferred, and CHPN is a plus.
