Job description
RVNAhealth seeks an experienced Hospice RN Case Manager to coordinate patient care across Fairfield and Litchfield Counties, delivering compassionate, individualized end-of-life care in patients’ homes and other settings.
You’ll lead an interdisciplinary team, conduct admissions and assessments, and develop plans of care with physicians, patients, and families, while ensuring compliant documentation and ongoing education for patients and caregivers.
