Job description
St. Croix Hospice is seeking a Registered Nurse Case Manager to provide in-home, holistic care and coordinate a personalized plan of care with patients and families. You will lead end-of-life discussions and ensure symptom control with compassion and clinical excellence.
This role requires RN licensure and at least 1 year of acute care experience, with hospice/home health preferred. You will rotate on-call duties and support care transitions, with benefits including health insurance and 401(k)
