Job description
St. Croix Hospice is seeking a Registered Nurse Case Manager to provide 1:1, whole-person care in patients' homes. You will coordinate individualized care plans with a multidisciplinary team and support families through complex decisions with empathy and expertise.
The role requires an RN with at least 1 year of acute care experience (hospice/home health preferred), on-call rotation, and reliable transportation. Starting salary ranges from $75,000 to $101,000, with comprehensive benefits.
