Job description
Gentiva Hospice is seeking a compassionate Registered Nurse to join our team as an RN Case Manager, delivering in-home hospice and palliative care. You will assess patients, develop plans of care, and coordinate with physicians, social workers, chaplains, and other professionals to ensure comfort and quality of life.
This role emphasizes education for patients and families, symptom management, and compassionate support through the end-of-life journey.
