Job description
Recooty is seeking a Hospice Registered Nurse (RN) Case Manager in Brookfield, WI to provide 40 hours per week of patient-centered care in home and community settings. The RN will assess, plan, implement, and evaluate care, coordinating with the interdisciplinary team to meet palliative needs.
Requirements include an active RN license, 1–2 years of acute care or home care experience, and a valid driver's license.
