Job description
Gentiva is seeking a dedicated Hospice RN / RN Case Manager to guide patients and families through end-of-life care in home settings. You will assess, plan, and implement personalized care plans with an interdisciplinary team, educate families, and ensure compassionate, compliant nursing support.
Responsibilities include conducting assessments, managing symptoms, coordinating visits, and maintaining accurate documentation while participating in on-call coverage as needed.
