Job description
St. Croix Hospice is seeking a Registered Nurse Case Manager to provide 1:1 holistic home-based care, partnering with an interdisciplinary team to create individualized care plans and guide end-of-life conversations with empathy.
You will educate patients and families on care, medications, and symptom management, while rotating on-call duties to ensure seamless transitions and quality care in the patient’s home environment.
