Job description
Our Hospice is seeking a Registered Nurse Case Manager (RNCM) to serve the Columbus team, visiting patients in their homes and facilities across Bartholomew County. You will coordinate with IDG members, manage caseloads, and ensure high-quality, compassionate care in a flexible Monday–Friday schedule with on-call rotation.
The ideal candidate holds an Indiana RN license (ASN/BSN preferred) with 1–3 years of healthcare experience, and the ability to travel using their own vehicle.
