Job description
Alabama Hospice Care, a part of LHC Group and part of the Optum family, invites you to join our team as a Registered Nurse Case Manager. You will deliver and oversee the Plan of Care for hospice patients, promoting physical, psychosocial, and spiritual well-being in collaboration with a skilled care team.
The role involves initial eligibility evaluation within 48 hours, ongoing assessments every 15 days, care coordination, accurate documentation, and patient/family education in home settings.
