Job description
Confidential Hospice Provider in North Texas seeks an experienced Hospice Registered Nurse Case Manager to deliver skilled nursing care and case management for patients across Denton County.
The RN Case Manager will assess needs, manage pain, coordinate the plan of care, and provide education and emotional support to patients and families on the hospice journey. This is a full‑time role with on‑call rotations and local travel.
