Job description
Gentiva Hospice seeks an RN Case Manager to lead in-home hospice care, guiding patients and families through the journey with compassion and expertise. You will perform comprehensive assessments, develop individualized plans, and coordinate with physicians, LPNs, CNAs, social workers, and chaplains to ensure comfort and quality of life.
