Job description
St. Croix Hospice in Minnesota seeks a Registered Nurse Case Manager to lead 1:1 holistic home-based care planning and family support. You will coordinate with an interdisciplinary team to develop individualized plans, guide end-of-life conversations, and ensure comfort with professionalism and empathy.
The role requires an active RN license with 1+ year acute care experience; hospice/home health background is preferred. On-call duties and reliable transportation are essential.
