Job description
Luminary Hospice is seeking a Registered Nurse Case Manager to lead patient-centered hospice care in home and community settings. You will perform comprehensive assessments, develop and revise plans of care, and coordinate services with the interdisciplinary team to meet patient goals and comfort needs.
