Job description
Center4Hospice in Elkhart, Indiana seeks an experienced Registered Nurse/Case Manager to join our compassionate hospice care team. This full-time, exempt role focuses on in-home patient care and coordinating plans with families.
You will conduct comprehensive assessments, develop individualized care plans, educate patients and caregivers, supervise LPNs and aides, and participate in on-call rotations with the interdisciplinary team.
