Job description
Anchor Health is seeking an experienced Hospice Registered Nurse Case Manager to join our team in California. The role involves conducting comprehensive patient and family assessments, developing and updating care plans, and performing home visits to monitor patient status.
The RN must hold a California license, CPR certification, and preferably 1–2 years of hospice/home care experience. A valid CA driver's license and insured vehicle are required.
