Hospice RN Case Manager — Coordinated Home Care

Job description

Affinity Health Management is seeking a Registered Nurse to plan, organize and direct hospice care in the community setting. The RN will base care on comprehensive assessments and collaborate with families to meet patient needs in home and community environments.

Responsibilities include care planning, nursing diagnoses, medication administration, patient/family education, and coordination with the interdisciplinary team, with emphasis on compassionate, patient-centered care.
