Job description
Gentiva Hospice seeks a dedicated Registered Nurse Case Manager to provide in-home hospice and palliative care. You will assess patients, develop and update plans of care, and coordinate with an interdisciplinary team to ensure compassionate, quality outcomes.
You will educate families, manage symptoms, document care, and participate in on-call coverage as needed. Valid RN license in New Jersey and CPR are required; hospice experience preferred.
