Job description
Agrace seeks a qualified RN Case Manager to deliver patient- and family-centered hospice care in the Sun Prairie, Deforest, and East Madison, WI areas. This full-time day-shift role includes on-call rotations, with a $6,000 sign-on bonus.
You will conduct assessments, manage plans of care, and educate families while coordinating with an interdisciplinary team. Ideal candidates hold an ADN or BSN and Wisconsin RN licensure, plus 1 year of nursing experience.
