Job description
Aurora Health Care seeks a Hospice Case Manager (RN) to coordinate patient and family care across hospice settings in Milwaukee. You will lead the IDG, develop and update the Plan of Care, and coordinate services to maximize quality of life at end of life.
The role requires an RN license, BLS, a valid driver’s license, and 1–2 years of nursing experience in home health, hospice or related fields. Strong communication and organizational skills are essential.
