Job description
Faith Hospice in Michigan seeks a Registered Nurse Case Manager to join its interdisciplinary team and deliver compassionate end-of-life care, coordinating medical and emotional support for patients and families.
The RN will work Monday–Friday daytime, participate in on-call rotations, and manage a caseload within a 50-mile radius around Grand Rapids, collaborating with social workers, chaplains, and aides.
