Job description
Beth Israel Deaconess Hospital Plymouth, part of Beth Israel Lahey Health, seeks an experienced Hospice RN Case Manager to join a collaborative interdisciplinary team delivering end-of-life care. You will coordinate patient plans, manage admissions and ensure appropriate documentation in the EMR, while supporting patients and families across home and facility settings.
The role requires an MA RN license with hospice/home health experience and a commitment to compassionate care.
