Job description
The Care Team Hospice is seeking a compassionate Hospice RN Case Manager to coordinate patient care, develop individualized plans of care, and collaborate with physicians and the interdisciplinary team to ensure comfort and quality of life.
You will perform skilled nursing visits, admissions, and symptom management using Homecare Homebase (HCHB), with on-call opportunities and a Monday–Friday schedule. Active RN license required; hospice experience preferred; generous benefits provided.
