Job description
Ohio Living is seeking an RN Case Manager - Hospice to plan, coordinate and deliver quality hospice care in accordance with company standards and regulations. Responsibilities include care management in homes and facilities, developing a care plan, maintaining accurate records, and ensuring regulatory compliance.
Requires Ohio RN license, 1 year acute care, proficiency with Windows and MS Office; EMR preferred; valid driver’s license and ability to travel.
