Job description
Partners In Recovery LLC seeks an experienced Registered Nurse Case Manager to join our hospice team in Elkhart, Indiana. This full-time, exempt role follows a Monday–Friday day shift with on-call rotation, providing comprehensive in-home care to patients and families.
You will assess needs, develop care plans, manage pain, educate families, and coordinate with the interdisciplinary team to ensure compassionate, quality end-of-life care.
