Job description
St. Croix Hospice seeks a Registered Nurse Case Manager to deliver 1:1, whole-person care in patients' homes. You will coordinate individualized plans with an integrated care team, supporting families through complex decisions with compassion and professionalism.
The role requires an RN from an accredited program and 1+ year in acute care; hospice/home health preferred. On-call duties and reliable transportation are required, with a commitment to dignity and comfort.
