Job description
Castle Senior Living, Inc. Hospice is seeking a Full Time Hospice Registered Nurse Case Manager (RNCM) to join our growing hospice program in Wisconsin.
The RNCM provides skilled nursing care at patients' homes, performs comprehensive assessments, develops and updates individualized plans of care, and coordinates with the Hospice Director. You will work with patients and families, maintain compassionate bedside care, participate in on-call rotation, and enjoy a friendly, family-owned workplace
