Job description
AMEDISYS HOME HEALTH is seeking a Hospice RN Case Manager to assess patients, coordinate care, and support families in the home setting. You will collaborate with the care team to implement individualized plans of care and maintain detailed progress records.
Requirements include a current RN license for the applicable state, CPR certification, and at least one year of nursing experience. A valid driver’s license and reliable transportation are required.
