Job description
Assisted Home Health & Hospice in Los Angeles is seeking a RN Case Manager for hospice services. This role is full-time preferred but open to per diem, supporting our LA hospice branch and coordinating patient care.
You will conduct initial and ongoing assessments, manage nursing care, educate families, and collaborate with clinical staff to deliver compassionate, compliant care close to home.
