Job description
Northern Arizona Hospice, part of the LHC Group within Optum's family, seeks a Registered Nurse Case Manager to deliver and coordinate hospice care under the Plan of Care. You will assess patients, develop and implement care plans, and collaborate with the IDG to promote quality, dignity, and support for patients and families.
Full-time role with flexible scheduling (3x12 or 4x10) and a $7,500 sign-on bonus. Requires active RN licensure in the state, CPR, and driving eligibility in Sedona, AZ.
