Job description
Buckeye Hospice and Palliative Care Services in Zanesville, Ohio is seeking a Registered Nurse (RN) Case Manager to deliver patient-centered, quality hospice care. You will assess needs, develop care plans, and coordinate services to promote independence and dignity.
The ideal candidate holds an active RN license, has 1-4 years hospice/home health experience, and is willing to participate in on-call rotations to support 24/7 coverage for patients and families.
