Job description
Compassus is seeking a Hospice Registered Nurse / RN Case Manager to provide compassionate, patient- and family-centered home-based hospice care. The role emphasizes collaboration with the care team to ensure comfort, dignity, and quality of life for patients while supporting families with education and planning.
Responsibilities include coordinating care, managing symptoms, and advocating for patient goals. A valid RN license, CPR/BCLS, and successful completion of onboarding are required.
