Job description
Hospice of North Alabama in Huntsville, AL is seeking a dedicated RN Case Manager to join our care team. You will assess patients’ needs, create care plans, and provide direct nursing care with a patient‑centered approach.
You will educate families, collaborate with an interdisciplinary team, maintain accurate documentation, and participate in on‑call rotations. A nursing degree with Alabama licensure and at least 1 year of nursing experience is required (3+ years preferred); hospice experience
