Job description
Caris Healthcare is seeking a Registered Nurse (RN) Case Manager to provide nursing care to patients per physician orders, assess and plan total patient care, and manage the overall plan of care for each patient on caseload. You will collaborate with families and an interdisciplinary team in a mission-driven hospice environment, serving patients across the Southeast region.
