Job description
RN Hospice Case Manager, Full Time
Providence at Home is a growing division of Providence Life Services, which provides a continuum of healthcare, focused on senior living. Providence Hospice is seeking a compassionate RN to join our established, experienced hospice team. We provide exceptional patient centered care to patients and families facing end of life. $5,000 retention bonus now being offered. Retention Bonus is paid out in increments when milestone hours are met.
Schedule Available: Monday - Friday. This position requires (1) on call night shift weekly.

Summary of essential job functions:
Provide skilled nursing care.
Provide case management services.
Conduct initial assessments of new patients.
Travel to patients wherever they reside: Home, nursing facility etc.
Requirements:
Active Registered Nurse (RN) License in the state of Indiana.
At least 1 year of RN experience, preferably in Hospice or Home Health.
Drivers License in good standing.
Reliable transportation.
