Job description
Phoenix Home Care & Hospice in St. Louis, MO is seeking an RN Case Manager to provide compassionate nursing care to clients facing terminal diagnoses.
You will collaborate with the interdisciplinary team to develop and implement patient- and family-centered plans of care and guide healthcare decisions to improve quality of life.
