Job description
Angels Care Hospice in McCook, NE is seeking a dedicated Registered Nurse Case Manager to coordinate all aspects of the patient’s home care and collaborate with other medical disciplines to provide professional, compassionate nursing services.
You will perform the admission visit in the patient’s home, determine eligibility for home care, and develop the plan of care to be followed, with strong emphasis on quality, safety, and continuous professional growth.
