Job description
Brighton Hospice is seeking a Registered Nurse Case Manager (RN) to plan, organize and direct hospice care across patient homes and communities in Indiana. The role emphasizes community health education, coordinating patient care, and building a support team with nurses, social workers, chaplains, volunteers, and CNA leadership.
You will assess patient needs, develop and adjust the care plan, administer prescribed treatments, and counsel families to promote comfort and quality of life.
