Job description
LSL ALF LLC is seeking a Hospice RN Case Manager to plan and direct patient care within the Northeast metro service area. This role requires conducting comprehensive patient assessments and developing care plans to meet individual needs.
The salary ranges from $88,000 to $94,600 annually based on experience, along with mileage reimbursement. Additional benefits include paid time off, life insurance, and a 401(k) plan.
