Job description
Peterson Health in Kerrville, TX is seeking a Hospice RN Case Manager to coordinate and direct hospice care in patients' homes and other settings. This full-time role runs Monday–Friday, 8am–5pm, and requires a strong background in end-of-life care, assessment, education, and collaboration with the provider and IDG members.
You will document thoroughly, participate in IDG and team meetings, provide on-call coverage, and support patients, families, and caregivers while adhering to hospice
