Job description
Visiting Nurse and Hospice for VT & NH seeks a per diem Hospice Registered Nurse to provide comprehensive in-home nursing care under physician supervision in Vermont and New Hampshire. The role includes home visit assessments, plan of care development, and coordination with the inter-disciplinary team.
