Job description
Chapters Health System seeks an RN Case Manager to assess patient/family needs, coordinate Plan of Care with the IDT, and deliver clinical, palliative, and supportive care to keep patients in their homes whenever possible.
The role requires an active Florida RN license, at least 1 year of nursing experience (hospice/hospital preferred), and proficiency with EMR/EHR systems. A valid driver’s license and reliable transportation are essential.
