Job description
Amedisys Hospice in Williamsburg, VA is seeking a dedicated RN Case Manager to assess patients, coordinate care with the care team, and implement individualized home care plans for patients in familiar settings.
Responsibilities include patient assessments, referrals, documentation, education for patients and families, and on-call duties. A current RN license, CPR, and a valid driver’s license are required, with at least one year of nursing experience.
