Job description
Gentiva Hospice is seeking a dedicated RN / Case Manager in Kansas to lead patients and families through the hospice journey at home or in supported settings. You will deliver skilled nursing care, create and update plans of care, and collaborate with physicians, LPNs, CNAs, social workers, and chaplains to ensure compassionate, personalized care.
You will educate caregivers, document thoroughly, and participate in on‑call and quality initiatives to support symptom management and end‑of‑life
