Tufts Medicine Care at Home is seeking a Hospice Registered Nurse, Case Manager in Malden, MA. The role focuses on delivering high-quality hospice nursing care, coordinating with the interdisciplinary team and updating care plans to reflect patient needs.
Strong assessment and communication skills are essential to support patients and families. The position requires an active RN license and hospice/home health experience; CPR is required, with BLS within 30 days if hired before 9/28/2026.
