Job description
St. Croix Hospice in Harrisonville, MO is seeking a Registered Nurse Case Manager to deliver individualized in-home care, coordinating with a multidisciplinary team to create and update plans of care for patients at the end of life.
You will lead meaningful conversations about goals of care, manage symptoms with clinical excellence, administer medications, educate patients and families, and participate in on-call duties to ensure seamless transitions.
