Job description
TN Quality Care Hospice in Waynesboro, TN seeks an RN Case Manager to manage a patient and family caseload, coordinate care and support home-based patients. The role involves nursing assessments, admission processing, plan implementation, and ongoing care evaluation.
This field-based position requires RN licensure, CPR/First Aid, and a valid driver's license. Schedule is Monday–Friday with occasional weekend on-call responsibilities, and a comprehensive benefits package is offered.
