Job description
Amedisys is seeking an experienced Registered Nurse to join our hospice team as a Case Manager. This full-time, first-shift role focuses on patient-centered care at home, coordinating with the care team to design and implement plans of care.
Responsibilities include conducting patient assessments, creating care records, coordinating referrals, teaching patients and families, and participating in on-call duties. Current RN license and CPR are required along with a valid driver’s license.
